A full investigation has been launched into police actions after a Wandsworth chase led to a crash in which a woman died and a man was seriously injured.

Acacia Smith, a 25-year-old childcare worker, was killed in the crash after police were called to reports of a drone being flown into Wandsworth Prison.

The driver, believed to be named Craig Kearney, remains in a serious condition in hospital after the Wandsworth Bridge Road crash at 3.04am on Wednesday.

The Independent Police Complaints Commission (IPCC) began investigating after being notified that police officers were following a Peugeot 206 in which Ms Smith was a passenger just before the car crashed into the front of a house.

IPCC investigators came to the scene to oversee the evidence gathering and interviewed officers about their involvement.

Neither of the police cars involved had in-car camera footage but local CCTV images are being gathered as well as information from the police car data recorders.

IPCC associate commissioner Tom Milsom said: "A young woman has died in terrible circumstances and our thoughts remain with her family, friends and all of those affected by this tragedy.

“Our investigation will examine police actions prior to the collision to determine whether appropriate policies and procedures were followed."
