After a slow start goals from Luis Cumbers and Jon Main saw the Dons into a comfortable 2-0 half-time lead.
Hornchurch, who finished fourth in the Ryman Premier League - a place behind Dons - pulled one back but Main sealed Wimbledon's place in the final with his second goal.
